Renewal Of Passport Appointment in the Philippines April-2022

Renewal of Passport is one of the periodicals and obligatory tasks for individual passport holders. To keep them updated on their records with the respective DFA (Department Of Foreign Affairs) offices. Whether at home or abroad, mostly avoid any travel and migration hassle.

Vital Requirements for Adult Renewal Applications:

  1. Confirm Online Appointment- To confirm an online appointment is the key -work to processing the renewal of a passport. Please (click here) to kick off. 

  1. Accomplished Application Form- Duly accomplished application form by filling all asterisk (*) fields.

  1. Personal Appearance- The applicant's personal- appearance is compulsory during passport renewal at scheduled DFA offices/branches. Whether at home or the foreign consulate office of the Philippines abroad.

  1. Current ePassport with bio-page photocopy- The applicant must bring a current ePassport with a bio-page photocopy. And to comply with DFA's entails surrendering thread of the current ePassport interim ongoing renewal process.   

  1. Original PSA authenticated documents might have supported the change of statistical status- If the applicant desire- off bring any further changes to either their civil status or any statistical- status. They must present the following authenticated supportive documents before DFA officials.  
        • PSA Marriage Contract original copy.

        • Annotated Birth Certificate Original Copy of PSA.

        • Annotated Marriage Contract to show annulment/divorce/court-ordered instruction.

        • Death Certificate of Spouse if Demised.

N.B: In case a woman applicant opts to preserve her maiden name. However, a Marriage Contract is not required. But additional requirements may be applicable.

Supporting Documents of ePassport for Adult Renewal Applications

With the core requirements, some additional following supporting documents will be further required by default cases:

N.B: In some default cases, the application is to be considered as a New Application rather than a renewal.

A Lost VALID ePassport - will be considered as a NEW APPLICATION:

If the holder lost their Valid ePassport will be considered a new applicant upon their applications. In such cases, the following procedures are to be maintained.

    • If the applicant's last issued passport photocopy is available, the Birth Certificate will not be required. But additional requirements may be applied. Please find appended below the Lost Valid ePassport requirements for the applicants.

          1. Police Report in English (Original and photocopy).

          1. Affidavit of Loss in English(Original and photocopy).

          1. An additional Penalty Fee: Of php350.00 is required.
    N.B: An additional 15 calendar days clearing period for Lost Valid Passports to be required.

    A Lost EXPIRED ePassport - will also be considered as a NEW APPLICATION:

    In case of a lost expired ePassport will also be considered a new applicant upon their applications. In such cases, the following procedures are to be maintained.

    • If the applicant's lost expired ePassport photocopy is available, the Birth Certificate will not be required. But additional requirements may be applied. Please find appended below the Lost expired ePassport requirements for the applicants.

          1. Affidavit of Loss in English(Original and photocopy).

          1. An additional Penalty Fee: Of php350.00 is required.

    A frequently Lost Passport renewal appeal - will also be treated as a NEW APPLICATION:

    In case of a frequently lost expired ePassport renewal appeal will also be considered a new applicant upon their applications. In such cases, the following procedures are to be maintained.

    • If the applicant's frequently lost ePassport photocopy is available, the Birth Certificate will not be required. But additional requirements may be applied. Please find appended below the frequently lost ePassport requirements for the applicants.

          1. Police Report in English (Original and photocopy).

          1. Affidavit of Loss in English(Original and photocopy).

          1. An additional Penalty Fee: Of php350.00 is required.

    • Unclaimed Passports were issued before the 2010 applicant, to be followed the procedures of a new application method.

    • Applicants with Unclaimed Passports issued from 2010 onwards-Unclaimed passport applicants issued from 2010 onwards may follow the appended bellows procedure.

        1. An applicant may request a Certificate of Unclaimed Passport either at the office where the unclaimed passport was issued, the nearest consular office, or at DFA Aseana.

        1. Upon obtaining the certificate, the applicant should set an appointment online through the DFA Passport Appointment website (www.passport.gov.ph). Courtesy lane eligible applicants may be exempt from setting an appointment online.

    In case of any applicant used a Travel Document to travel back to the Philippines - further would be treated as a NEW APPLICATION:

    In case of any applicant used a Travel Documents to travel back to the Philippines ePassport renewal appeal will also be considered a new applicant upon their applications. In such cases, the following procedures are to be maintained.

    • If the applicant's last issued Passport photocopy is available, the Birth Certificate will not be required. But additional requirements may be applied. Please find appended below the requirements for the applicants.
    Additional Requirements for Applicants who used Travel Documents to Travel back to the Philippines.

          1. Original Travel Document.

          1. Affidavit of Explanation.

    In case of any applicant used a Travel Document to travel back to the Philippines Please present the following supporting documents.

          1. Original Travel Records from Bureau of Immigration.

          1. Affidavit of Explanation.

    In case the passport has been damaged or mutilated - also will be treated as a NEW APPLICATION.

    In case the Passport has been damaged or mutilated renewal appeal will also be considered as a new applicant upon their applications. In such cases, the following procedures are to be maintained.

    • If the applicant's last issued Passport photocopy is available, the Birth Certificate will not be required. But additional requirements may be applied. Please find appended below the requirements for the applicants.

    Additional Requirements for damaged or mutilated passport applicants.

          1. Affidavit of Explanation.

          1. Applicant must surrender mutilated/damaged passport.

          1. A penalty Fee: of Php350.00 is required.

    For dual citizen applicants, the following additional supporting documents are- required:

    1. To confirm Online Appointment (click here).

    1. Government Issued IDs (Refer to the list of valid Philippine Government Issued IDs or their secondary citizenship/residence counterparts).

    • If the applicant possesses dual citizenship from birth ( one parent is a Filipino while the other parent is a citizen from which the applicant's other citizenship is- derived, or both Filipino parents but were born in a country which applies jus soli citizenship; etc.)- An original and a photocopy of PSA-authenticated Birth Certificate/Report of Birth is required.

    • If the applicant's Filipino citizenship was reacquired or retained as specified in RA 9225-An original and a photocopy of identification certificate issued by a Philippine Foreign Service Post (FSP) or by the Bureau of Immigration (BI)

    If the applicant is a Naturalized Citizen: 

    If the applicant is a Naturalized Citizen Identification Certificate of Naturalization from the Bureau of Immigration additional core documents are required. 

    In case the applicant is a Filipino citizen by Election:

    If the applicant is a Filipino citizen by Election the following supporting documents are required with the applications.

          1. Affidavit of Election of Philippine Citizenship.

          1. Identification Certificate of Election from BI.
